          Community Health Systems Chaplain/Pastor/Spiritual Advisor-Hospice in Wasilla Alaska United States

          Title: Chaplain/Pastor/Spiritual Advisor-Hospice
          Location: AK-Wasilla
          · The spiritual care coordinator is responsible for the provision of spiritual care services to patients/families/caregivers on Hospice directly and through coordination of care with other spiritual counselors.

          · Performs other related duties as assigned. Position purpose statements are intended to describe the general nature of work to be performed and are not intended to be construed as an exhaustive list of all responsibilities. Management reserves the right to reassign duties as needed.

          The Chaplain/Pastor/Spiritual Advisor performs duties in accordance of and under the direction/supervision as defined by the Agency's organizational chart.

          Education: Hospice Training, must be able to demonstrate the level of knowledge and skills normally associated with a Bachelors Degree, including knowledge of Hospice, death, dying, grief and bereavement.

          Experience: Experience working with death and dying, individuals/family/caregiver. Ability to work as a member of interdisciplinary team. Comfort in an interfaith setting. Ability to accept different lifestyles, cultures, beliefs, and value. Ability to network with community clergy and congregations. Knowledge of and commitment to Hospice philosophy of care. Competent to provide Spiritual services to Geriatric and Adult patients. Hospice experience. Able to communicate well verbally and in writing, problem solve, manage crisis situations, and effect change. Able to travel to patient/family homes. Able to document appropriate patient information.

          Licenses/Certificates: Driver's license, CPR, Bachelors of Arts. Primary Locaiton: AK-Wasilla
